Case Analysis – Wentworth Medical Center

As part of a long-term study of individuals 65 years of age or older, sociologists and physicians at the Wentworth Medical Center in upstate New York investigated the relationship between geographic location and depression. A sample of 60 individuals, all in reasonably good health, was selected; 20 individuals were residents of Florida, 20 were residents of New York, and 20 were residents of North Carolina. Each of the individuals sampled was given a standardized test to measure depression. The data collected follow; higher test scores indicate higher levels of depression. These data are contained in the file Medical1.

A second part of the study considered the relationship between geographic location and depression for individuals 65 years of age or older who had a chronic health condition such as arthritis, hypertension, and/or heart ailment. A sample of 60 individuals with such conditions was identified. Again, 20 were residents of Florida, 20 were residents of New York, and 20 were residents of North Carolina. The levels of depression recorded for this study follow. These data are contained in the file named Medical2.

Case Study – Measuring Stock Market Risk

One measure of the risk or volatility of an individual stock is the standard deviation of the total return (capital appreciation plus dividends) over several periods of time. Although the standard deviation is easy to compute, it does not take into account the extent to which the price of a given stock varies as a function of a standard market index, such as the S&P 500.

As a result, many financial analysts prefer to use another measure of risk referred to as beta. Betas for individual stocks are determined by simple linear regression. The dependent variable is the total return for the stock and the independent variable is the total return for the stock market. For this case problem we will use the S&P 500 index as the measure of the total return for the stock market, and an estimated regression equation will be developed using monthly data. The beta for the stock is the slope of the estimated regression equation (b1). The data contained in the file named Beta provides the total return (capital appreciation plus dividends) over 36 months for eight widely traded common stocks and the S&P 500.

The value of beta for the stock market will always be 1; thus, stocks that tend to rise and fall with the stock market will also have a beta close to 1. Betas greater than 1 indicate that the stock is more volatile than the market, and betas less than 1 indicate that the stock is less volatile than the market. For instance, if a stock has a beta of 1.4, it is 40% more volatile than the market, and if a stock has a beta of .4, it is 60% less volatile than the market.

Overview

For this assignment, you will assess and analyze a fictitious client based on a theory of psychotherapy of your choice. This will be the first half of a case conceptualization to be completed for the final assignment. Think of this assignment as part of the first stage of therapy where you are getting to know your client using a theory of psychotherapy AND Stage 1 of the Egan and Reese Skilled Helper Model to guide how you make sense of the client’s story and their reasons for seeking help.

    Instructions

    Address the elements to be included in the first half of the Case Conceptualization in bold found below. You may wish to use these elements as both an outline for your paper and actually the headings to guide its organization and development. The Case Conceptualization – Part 1 should include the following:

    Introduction

    Note: Do not include the word Introduction as a heading. APA instructs us that the content is implied due to its position in a paper. Instead, begin your paper with the content explained below. See APA Publication Manual for additional details on this topic.

    • Include a general introduction to your paper orienting your reader to the topics and information that will be covered within the report.
      • This introduction is not to be confused with the background of the client; instead the introduction referred to here is to inform your reader what the paper is about. Be sure to include that the client being discussed is fictitious. This is to ensure there is never a question around practicing without a license or keeping the APA Code of Ethics on your radar.

    • Application of Theory: Explain the theory of psychotherapy you have selected to apply to this case. Identify the basic tenets. Dr. Ellis would challenge his client to identify and dispute irrational beliefs. When an irrational belief is changed, often times emotional and/or behavioral responses to external stimuli will also change.
      • Explain how and why the problem developed using the theory of psychotherapy you chose to understand this client. This is your conceptualization within the context of your theory.
      • Assess the client’s strengths and difficulties, applying the theory of psychotherapy chosen for the project. State your hypotheses about the client’s personality and functioning. What are the client’s abilities and resources? What are the client’s deficits, pathology, or issues?

    Volkswagen had a major push to sell diesel cars in the US, backed by a huge marketing campaign trumpeting its cars’ low emissions, the Environmental Protection Agency (EPA) discovered in 2014 that Volkswagen had deliberately installed cheating software in more than 500,000 diesel passenger cars sold in the United States. The software allowed the cars’ computers to sense when the vehicles were being tested for harmful emissions and to switch, undetected, to a driving mode that artificially reduced nitrogen oxide or NOx (linked to smog and lung cancer) so the cars could pass the test. Once back on the road, the computers adjusted the driving mode again, permitting drivers to experience the power and mileage the company was touting while allowing some cars to emit as much as 40 times the volume of pollutants permitted by law. Volkswagen had done the same in more than 10 million other cars sold around the world. The U.S. government’s case against VW resulted in a settlement of almost $15 billion and included massive repair and buy-back programs. Another $4.3 billion in civil and criminal penalties was levied against the company, some of which is to fund projects to reduce NOx in the air; eight high-ranking executives were charged with crimes, and two received prison sentences. The company lost more than a quarter of its market value as its stock price plummeted. How did a venerable and successful company come to commit what one business publication called “a truly brazen act of business malfeasance” in the first place? More stringent controls on NOx took effect in 1999, and of the two possible ways it could meet them, VW chose the one that did not require a costly reengineering of the interior of its cars. This might have seemed like the right business decision to make, but as a side effect, VW’s new NOx controls dramatically reduced the degree of fuel economy the company was advertising. It was apparently at this point that the decision to rig the cars to falsify emissions tests was made. Volkswagen’s financial performance has returned to pre-scandal levels even as the possibility remains that more huge fines will be levied, this time by European investigators. In early 2018, another scandal broke when the company was forced to apologize for exposing monkeys to engine fumes to research the effects of diesel exhaust. “We’re convinced the scientific methods chosen then were wrong,” said a company statement. “It would have been better to do without such a study in the first place.”

    A large fast-food chain unveiled a new
    touch screen register for its franchises. Each cashier was assigned a

    Salim came out of Mrs. Zeenat’s cabin and went straight to where his team members sat. He waited for the others to come up with their suggestions but not one of them spoke up. He was surprised, and asked them point-blank why they were so disinterested.

    Faisal, one of the team members, said, “What is the point in our discussing these things? Mrs. Zeenat is not going to have time to listen to us on discuss anything. She will just give us the solution she thinks is best, and we will just do what she tells us to do; why waste everyone’s time?”

    Salim felt his heart sink. Was this the way things worked over here? However, he refused to lose heart and thought that maybe, he could change things a little. But as the days went by, Salim realized that Mrs. Zeenat was the complete opposite of his old boss.

    While she was efficient at what she did and extremely intelligent, she had neither the time nor the inclination to groom her subordinates. Her solutions to problem were always correct, but she was not willing to discuss or debate the merits of any other ideas that her team might have. She did not hold the team down to their deadlines not did she ever interfere. In fact, she rarely said anything at all. If work did not get finished on time, she would just blame her team, and totally disassociate herself from them.

    Time and again, Salim found himself thinking of Abdulrahman his old boss, and of how he had been such a positive influence. Mrs. Zeenat, on the other hand, even without actively doing anything, had managed to significantly lower his motivation levels.

    Salim gradually began to lose interest in his work – it had become too mechanical for his taste. He didn’t really need to think; his boss had all the answers. He was learning nothing new, and he felt his career was going nowhere. As he became more and more discouraged, his performance suffered. From being someone with immense promise and potential Salim was now in danger of becoming just another mediocre techie.

    When taxes induce people to change their behaviour—such as inducing Jane to buy less pizza—the taxes cause deadweight losses and make the allocation of resources less efficient. As we have already seen, much government revenue comes from the individual income tax in many countries. In a case study in Chapter 8, we discussed how this tax discourages people from working as hard as they otherwise might. Another inefficiency caused by this tax is that it discourages people from saving.

    Consider a person 25 years’ old who is considering saving $1,000. If he puts this money in a savings account that earns 8 percent and leaves it there, he would have $21,720 when he retires at age 65. Yet if the government taxes one-fourth of his interest income each year, the effective interest rate is only 6 percent. After 40 years of earning 6 percent, the $1,000 grows to only $10,290, less than half of what it would have been without taxation. Thus, because interest income is taxed, saving is much less attractive.

    Some economists advocate eliminating the current tax system’s disincentive toward saving by changing the basis of taxation. Rather than taxing the amount of income that people earn, the government could tax the amount that people spend.

    Under this proposal, all income that is saved would not be taxed until the saving is later spent. This alternative system, called a consumption tax, would not distort people’s saving decisions.

    Various provisions of the current tax code already make the tax system a bit like a consumption tax. Taxpayers can put a limited amount of their saving into special accounts—such as Individual Retirement Accounts and 401(k) plans—that escape taxation until the money is withdrawn at retirement. For people who do most of their saving through these retirement accounts, their tax bill is, in effect, based on their consumption rather than their income.

    European countries tend to rely more on consumption taxes than does the United States. Most of them raise a significant amount of government revenue through a value-added tax, or a VAT. A VAT is like the retail sales tax that many U.S. states use, but rather than collecting all of the tax at the retail level when the consumer buys the final good, the government collects the tax in stages as the good is being produced (that is, as value is added by firms along the chain of production). Various U.S. policymakers have proposed that the tax code move further in direction of taxing consumption rather than income.     Chronic obstructive pulmonary disease (COPD) is a preventable and treatable disease characterized by persistent and progressive airflow limitation. Clinical manifestations typically develop slowly, beginning with a chronic, intermittent cough and can progress to dyspnea at rest in the late stages.

    “Mrs. Walsh, a woman in her 70s, was in critical condition after repeat coronary artery bypass graft (CABG) surgery. Her family lived nearby when Mrs. Walsh had her first CABG surgery. They had moved out of town but returned to our institution, where the first surgery had been performed successfully. Mrs. Walsh remained critically ill and unstable for several weeks before her death. Her family was very anxious because of Mrs. Walsh’s unstable and deteriorating condition, and a family member was always with her 24 hours a day for the first few weeks.
    The nurse became involved with this family while Mrs. Walsh was still in surgery, because family members were very anxious that the procedure was taking longer than it had the first time and made repeated calls to the critical care unit to ask about the patient. The nurse met with the family and offered to go into the operating room to talk with the cardiac surgeon to better inform the family of their mother’s status.
    One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
    The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. She believes that a developmental process is involved when nurses learn to work with families.
    She noted that after a nurse has lots of experience and feels very comfortable with highly technical skills, it becomes okay for family members to be in the room when care is provided. She pointed out that direct observation by anxious family members can be disconcerting to those who are insecure with their skills when family members ask things like, “Why are you doing this? Nurse ‘So and So’ does it differently.” She commented that nurses learn to be flexible and to reset priorities. They should be able to let some things wait that do not need to be done right away to give the family some time with the patient. One of the things that the nurse did to coordinate care was to meet with the family to see what times worked best for them; then she posted family time on the patient’s activity schedule outside her cubicle to communicate the plan to others involved in Mrs. Walsh’s care.
    When Mrs. Walsh died, the son and daughter wanted to participate in preparing her body. This had never been done in this unit, but after checking to see that there was no policy forbidding it, the nurse invited them to participate. They turned down the lights, closed the doors, and put music on; the nurse, the patient’s daughter, and the patient’s son all cried together while they prepared Mrs. Walsh to be taken to the morgue. The nurse took care of all intravenous lines and tubes while the children bathed her. The nurse provided evidence of how finely tuned her skill of involvement was with this family when she explained that she felt uncomfortable at first because she thought that the son and daughter should be sharing this time alone with their mother. Then she realized that they really wanted her to be there with them. This situation taught her that families of critically ill patients need care as well. The nurse explained that this was a paradigm case that motivated her to move into a CNS role, with expansion of her sphere of influence from her patients during her shift to other shifts, other patients and their families, and other disciplines”

She had been suffering from emphysema for several years and had had frequent hospitalizations for respiratory problems. Upon consultation with the vice president for nursing, the nurse manager and the unit staff nurses decided against moving Mrs. Smith to the palliative care unit, although considered more economical, because of the need to protect and nurture her because she was already experiencing signs and symptoms of the dying process. Nurses were prompted by an article they read on human caring as the “language of nursing practice” (Turkel, Ray, & Kornblatt, 2012) in their weekly caring practice meetings.
    The nurse manager reorganized patient assignments. She felt that the newly assigned clinical nurse leader who was working between both the medical and surgical units could provide direct nurse caring and coordination at the point of care (Sherman, 2012). Over the next few hours, the clinical nurse leader and a staff member who had volunteered her assistance provided personal care for Mrs. Smith. The clinical nurse leader asked the nurse manager whether there was a possibility that Mrs. Smith had any close friends who could “be there” for her in her final moments. One friend was discovered and came to say goodbye to Mrs. Smith. With help from her team, the clinical nurse leader turned, bathed, and suctioned Mrs. Smith. She spoke quietly, prayed, and sang hymns softly in Mrs. Smith’s room, creating a peaceful environment that expressed compassion and a deep sense of caring for her. The nurse manager and nursing unit staff were calmed and their “hearts awakened” by the personal caring that the clinical nurse leader and the volunteer nurse provided. Mrs. Smith died with caring persons at her bedside, and all members of the unit staff felt comforted that she had not died alone.
    Davidson, Ray, and Turkel (2011) note that caring is complex, and caring science includes the art of practice, “an aesthetic which illuminates the beauty of the dynamic nurse-patient relationship, that makes possible authentic spiritual-ethical choices for transformation—healing, health, well-being, and a peaceful death” (p. xxiv). As the clinical nurse leader and the nursing staff in this situation engaged in caring practice that focused on the well-being of the patient, they simultaneously created a caring-healing environment that contributed to the well-being of the whole—the emotional atmosphere of the unit, the ability of the clinical nurse leader and staff nurses to practice caringly and competently, and the quality of care the staff were able to provide to other patients. The bureaucratic nature of the hospital included leadership and management systems that conferred power, authority, and control to the nurse manager, the clinical nurse leader, and the nursing staff in partnership with the vice president for nursing. The actions of the nursing administration, clinical nurse leader, and staff reflected values and beliefs, attitudes, and behaviors about the nursing care they would provide, how they would use technology, and how they would deal with human relationships. The ethical and spiritual choice making of the whole staff and the way they communicated their values both reflected and created a caring community in the workplace culture of the hospital unit.

In addition to its medical promise, the drug also has the potential to be a huge profit maker for the company.

    The problem lies in the fact that the substance for the treatment comes from the bark of the Pacific Yew Tree, a species that grows in the Pacific Northwest and has fallen victim to clear cutting, making them scarcer than they once were. Environmental groups are concerned that cutting down more of the trees will negatively impact sensitive ecosystems, habitat for other wildlife, and biodiversity (prompting questions like “what other wonder cures/ drugs might be destroyed if the forest is altered.”)

    Researchers and company executives have tried to come up with a way to sustainably harvest the trees to treat the 12,500 women (at the time) who die of ovarian cancer each year.     In doing your cases you will need a minimum of five references outside the material given you with the case. Use current business websites to find out about recent developments.

    The case study analysis allows you to apply your knowledge to the real world. Your goal is to identify a major problem confronting the subject company and provide a strategic solution for the problem. You will need to collect data and interpret it. You must also isolate the critical issues that the company faces. Remember that in the words of Lord Kelvin, “anything that cannot be expressed in numbers represents knowledge that is of a poor and uncertain kind.”

    After identifying the critical issues you can generate alternatives to address the company’s competitive situation. Evaluating these alternatives allows an organization to select one course of action. With a course of action defined, the strategic manager can then provide a plan for implementation. This is what you are to do on the case studies. Always refer to the rubric for the critical points to cover.

    The format for the case studies should be as follows.

    1. APA cover page,
    2. One-page, single spaced Executive Summary for the case as noted below. This is a summary of your report and, therefore, must be done after you finish the report. This one-page report must use headings and subheadings indicated in the following format to identify the critical issues. Begin with your understanding of the situation in a problem statement. Follow this with a concise presentation of your analysis and the alternatives you see. Recommend one course of action and support that recommendation with facts and other information, such as competitor’s moves. The last item is presents a plan for implementation –a sentence or two is sufficient. A sample of the Executive Summary is available with all this material.
    3. Body of your report. The summary page should encapsulate your understanding to the problems facing the company, a brief summary of the strategic analysis, financial analysis, the alternatives considered and a single recommendation with an implementation plan.

    After the executive summary page, you are to use standard APA formatting for a 5-page report. Tables, charts, graphs, etc. are to be put in the appendix and referenced from the body of your paper. Deductions may be taken for papers where the body is longer than five pages. Remember, in management you must get your thoughts across quickly if you expect your work to be read. Long reports generally wind up in a stack awaiting reading at some future date (which never comes).

    Case Study Report Format:

    Your report must include:

    1. NSU Cover page (1 page in length). It will present the name of the case and the author(s) of the report.
    2. Executive Summary (1 page in length).

    The Executive Summary is a concise overview of the report. The Case Study Report should be written from the perspective of an outside consultant, writing to the Board of Directors of the firm. It notes the essential points of the report and must have the following sections:

    1. Problem Statement: State the main problem facing the firm (or industry) in one, succinct sentence.
    2. Analysis: Summarize the main findings of your analysis. You may use bullet points, bold, italics – any means to convey and highlight the key factors you have determined based on your analysis. Don’t repeat items from the body of your report like the SWOT. Summarize the major issues.
    3. Alternatives: State briefly (one sentence or a bullet point each) 2 or 3 alternative courses of action that could be implemented
    4. Recommendation: Choose one course of action and support your choice.
    5. Implementation: Briefly (1 or 2 sentences are sufficient) present how the plan would be implemented. This tests the viability of the choice. For example, your plan would demonstrate that the company has the people, financial resources and time to implement your recommendation.

    These bullets should appear in your paper. The following is a precise format for the Case Studies.

    Case Study Preparation

    The preparation of the cases in this course is different than the cases you have written in the past. These will require substantial investigation into the case company’s strategies, competitive positions and actions, problems being faced, the company financials, ratios, and trends. The case studies are about a real company’s current position in the industry, the external competitive environment, the current strategies being used, and recommendations for strategies the company should use going forward. The cases should identify a problem faced by the company, using the strategic management tools highlighted in the text and quantitative analysis. You then generate realistic solutions, evaluate and select one, and then provide recommendations and the timeline to implement your recommendations. Remember that you are working to understand the company’s current strategy and future direction that offers a solution to the problem of meeting the company’s shareholder requirements. Again, remember that you are fulfilling the requirements of the rubric for each of these cases.>

    However, in a case study you have to look at a company’s performance over the past few years and understand how their performance is changing and identify underlying problems. You are to identify trends in performance, problem areas, and how the company is performing using all of the tools that you have learned throughout your MBA program. Remember that you are working to meet or exceed the shareholder expectations.

    The perspective for your report analysis should be directed to the company’s Board of Directors (BoD). Remember that the BoD is familiar with the company’s history, its management structure and the strategies deployed in the past. Recycling old history about the formation of the company, its previous management, etc. will not be looked upon favorably by the BOD (or your instructor).
